RN Case Manager:

This registered nurse will be visiting you routinely in addition to as needed. The RN Case Manager coordinates all aspects of a patient’s care. This nurse is your advocate and will make sure all your needs are met. They manage symptoms, communicate with the care team, support families, and ensure each patient’s comfort and wishes are honored throughout their journey. It is our hope that you see your nurse as “part of the family”!

Health Aide:

Provides personal care and emotional support to patients. They assist with daily activities such as bathing, grooming, dressing, and mobility, while monitoring the patient’s comfort and reporting changes to the RN Case Manager. Their goal is to promote dignity, safety, and compassion for both the patient and family.

Hospice Doctor:

Oversees the medical care of hospice patients. They manage symptoms, prescribe medications, and guide the care team in providing comfort-focused treatment. Their goal is to ensure dignity, peace, and quality of life for each patient and family. They can also provide care conferences when necessary.

Spiritual Counselor:

Provides guidance and presence for patients and families seeking comfort, peace, and meaning—honoring each person’s faith, values, and beliefs. You do not have to be religious to benefit from a visit from our Spiritual Counselor. This person can be there for you to provide a wide range of support. Some patients enjoy a visit from our spiritual counselor to pray or hear a spiritual story while others simply just want someone who will listen, play music or offer a hand to hold.

LVN:

Provides skilled nursing care under the direction of the hospice RN and physician. They monitor symptoms, administer medications, and ensure patients’ comfort while offering support to families throughout the care journey.

Bereavement Coordinator:

Supports families through grief and healing after loss by offering counseling, memorial resources, and ongoing emotional support. Aster continues to provide care to your loved ones to make sure they have the support they need.

Volunteers:

Offers companionship, comfort, and a caring presence to patients and families—helping with errands, listening, or simply sharing time together. Our Volunteers as Aster Health go above and beyond to assist all disciplines by finding ways to bring light and cheer to our community. They devote their time, creativity and innovation to employees, patients and caregivers. They are the true heart of hospice.

Nurse Practitioner:

Delivers advanced clinical care focused on comfort and quality of life. They assess patients, manage symptoms, prescribe medications, and collaborate with the hospice team to ensure individualized, compassionate care.

Social Worker:

Supports patients and families with emotional care, counseling, and practical assistance. They help navigate end-of-life decisions, connect to community resources, and promote peace and understanding during the hospice journey. They are incredible resources and specialize in educating you and your family to enable you to make the best decisions along the way.

Admissions Nurse:

This is a registered nurse who guides patients and families through the start of hospice care. They assess medical needs, explain services, complete necessary documentation, and ensure a smooth, compassionate transition into hospice support while communicating directly with the hospice physician.

This team surrounds you (like petals on a flower) to cultivate a warm and compassionate environment that assures all your needs are met.
